The ambulance version is a station wagon. In order not to stand out against the background of the government motorcade, all such vehicles were painted black, for which they received the nickname "Black Doctor". Contrary to popular belief, the equipment of this vehicle did not meet the requirements of an ambulance. It was intended only to transport high-ranking patients in case of forced hospitalization. The vehicle was designed for a team of three doctors and one bedridden patient. The length of the station wagon body was increased, and a superstructure was installed over the medical part of the cabin. The rear door consisted of two sections: the upper one rose, the lower one folded down. The weight of the vehicle was 3.9 tons. The rear left door was false, behind it was a spare wheel and tools. The interior of the vehicle was finished to the highest class, even the stretcher was made of genuine leather. The medical station wagons created later on the basis of the ZIL-41045 and ZIL-41047 had the same designation - ZIL-41042. The vehicles built on the ZIL-41047 basis were distinguished by a solid rear door that lifted upwards. A total of four ambulances were manufactured on the ZIL-4104 basis and its modifications.
